Edit the Reviews Form:
Navigate to Toolbox - Form Manager look for the form: Listing - Write Review
Hover over 'Actions' and click on Customize:
The Title and Review fields can be edited inside the Reviews Form. Bear in mind that the option to make the Title field required will not display, due that this field will always be required.
The email address is a mandatory field. Even though the form has the option to turn off the required field, the reviews feature is coded to require this field always.

Edit the Rating Criteria
The Reviews form also includes a list of criteria to rate the member:
The rating criteria can be edited through the text labels. Go to:
Settings > Text Labels > search for "rating criteria":
IMPORTANT: If one of the 6 labels is left blank, it will not appear on the form:
